OUR RESPONSIBILITIES

Al Yousif Hospital is committed to maintaining your medical records while keeping your health information private and secure in accordance with Ministry of Health regulations.


YOU HAVE THE RIGHT TO ASK FOR A COPY OF YOUR MEDICAL RECORDS

Although your medical record data belongs to the hospital, the information contained in the record belongs to you. If you would like us to forward a copy of your medical record, to another healthcare facility/provider, insurance company, attorney or another individual, we need your written authorization and a legible copy of your valid photo identification.


HOW IS MEDICAL INFORMATION USED?

Al Yousif Hospital uses medical information for patient care, to plan care and treatment, to carry out routine health care functions, and for payment purposes.


WHEN CAN I GET MY REPORTS?

If the report has been typed and signed, it can be printed immediately from the Medical Record From the time of request, it could take up to 24 hours for the patient's report to be ready for release. REPORTS CAN NOT BE RELEASED UNTIL THEY HAVE BEEN SIGNED BY THE PHYSICIAN. Release of Information is open from 09.00 am until 08.00 pm Saturday through Thursday.


WHY DO I HAVE TO SIGN A CONSENT FORM?

When a patient signs the Consent for Release of Information (MR-FO-62), they are giving a permission to release or disclose (provide to others) health information. All patients must authorize the release of their medical information and highlight to whom this information can be released.


CAN I HAVE A REPRESENTATIVE RETRIEVE MY RECORDS?

Records can be released to a representative, spouse, or family member if the representative, spouse, or family member has a signed authorization from the patient authorizing the release. The representative, spouse, or family member must present a valid ID (Iqama copy or passport copy or Saudi ID) and a copy of a valid ID from the patient before the information can be released. Records of minor children can be released to the parents without an authorization. The parent will need to present their ID to receive the records of their minor child or children.


CAN I HAVE MY RECORDS FAXED OR EMAILED?

Because of our obligation to protect patient confidentiality, we cannot:

  • e-mail patient information
  • fax patient information to a private residence
